Access G-AP is an accessible version of the G-AP record and includes a G-AP manual and a G-AP logbook. It was developed in consultation with people with communication difficulties and rehabilitation staff.

Access G-AP uses supporting images and accessible formatting. Access G-AP is most useful with people with cognitive and/or communication difficulties but can also be used with anyone who may benefit from additional support with understanding the goal setting process.

The G-AP record/Access G-AP should be held with the person you are setting goals with so that they are able to have access to their own goals and plans. This encourages the person to take ownership of their goals and they can also share them with family and friends. It may be helpful to keep all of the paperwork together in one easily accessible folder. If people would prefer you to keep the G-AP record for them, that’s OK too.

The G-AP resources can be modified to suit the individual’s needs. For example, there are options to include multiple or single page goals and action plans depending on how much space is needed. There are also G-AP prompt cards and single stages of the G-AP process on cards which may help to facilitate your conversations with people, or to help remind you of the different stages of G-AP.
