Appraisal
What?
Together, the staff member and person consider the outcome of the action plan and judge whether progress is being made (or not) towards achieving the goal.
How?
The staff member asks the person how they got on carrying out the plan.
As well as understanding the person’s perspective about how they got on, the staff member makes their own assessment of how the person got on carrying out the plan. This might be based on what the person says, looking at the end result of their actions or by watching by them actually doing it.
Why?
It’s important to understand the person’s perception of how they got on; this will influence the feedback given in the next stage.
This stage creates an opportunity for the staff member and the person to review each action plan as they are completed and decide whether progress is being made towards reaching the goal.
If the person has not attempted to complete agreed plans, this is a good opportunity to explore why.
Outcome of this Stage
- The staff member together with the person will have considered the outcome of the action plan and judged to what extent it has helped them make progress in reaching the goal.
- If the person has not attempted to complete the plan as agreed, this can be addressed and managed in a timely way.

In this example, the person was happy to tell the staff member that she successfully completed the plan – so ‘Yes’ is circled to document successful completion of the plan.
